Weight Loss Readiness Quiz

Does anyone have a weight loss readiness quiz that I would be able to

use in something that I am ghost writing. I would be happy to add a

source. I need something I can write out, not something that they

would take online.

I did upload 2 items to RD-USA. The email came out about it and did not

lead to the correct information. So, I went in and started a NEW FILE - Weight

Loss Readiness. I then uploaded 2 items.
