What is efficient and effective really depends on so many factors, but I agree with Jan that a walking program may be a good place to start. Maybe initiatives to increase fruit and vegetable consumption. The best place to start is to listen to people in your community. Unfortunately a lot of great ideas get discarded simply because they weren't designed to handle the dynamics at the local level. What state are you in? Check with your state's division of public health to see if there are any initiatives you can partner with. If there is a public health program at a university in your state, you may be able to talk with someone there too.
