I can think of several patients who have continually changed MDs to secure a tx eval/tx order! One way we've tried to manage same is to provide the patient with a MC denial notice, as per WI FI something like 'The provider should have known that services were non covered'-which in denail of services comes to us in these sceanrios. Post denial MC (A/B) we provide another letter to the pt/DPOA re: options and allow 'selection' of choice, then document same. This includes the therapist screen and/or restorative aspects per eval-to-restorative, whatever history we have/are aware of, and that the skilled aspect appears to be unwarranted. The therapist contacts the ordering MD with same (does NOT necessarily stop the MD from ordering/reording services in short sequence...we try to educate the docs re: guidelines). Hope this helps!
