I encourage people to report any serious adverse events (such as severe

worsening of symptoms, associated in time with a person taking a prescription

or over-the-counter medication, or a nutritional supplement, to the FDA Medwatch

program. You don't have to name the doctor, or know for sure that the adverse

event was caused by the medication or supplement. The FDA looks for patterns in

the reports to determine if additional studies or labeling are needed.

If a medication error (such as the wrong drug or dose) occurs as a result of

pharmacy error, you may notify your State Board of Pharmacy.
