You don't have to name the doctor, or know for sure that the adverse event was caused by the medication or supplement. The FDA looks for patterns in the reports to determine if additional studies or labeling are needed. If a medication error (such as the wrong drug or dose) occurs as a result of pharmacy error, you may notify your State Board of Pharmacy.
