Regarding Acute Care Discharges

I would like some input as to a reference for writing a formal discharge summary

on patients who have been discharged from the hospitas by the physician before

all goals were achieved in PT.

My feeling is this patient does not need a formal discharge summary from PT

because PT did not discharge them from service the physician's discharge summary

explains why.

Could someone please site me a reference if my conclusion is wrong.
