Medics could be individually placed with their areas of interest and proficiencies. Patients would lose some of the assembly line impersonal transfer of care until they go to the area of tertiary care and I fail to see why medics could not follow up with home visits after discharge. On many levels this would cement us as the emergency paramedics and limit the view of van drivers or ambulance guys.
