Helen:

The G0402 is the IPPE. Think only in 1st 12 months of medicare and follow the CMS template for the IPPE.

Ther G0438 is the AWV. The first AWV can be given after 12 months coverage of medicarewith or without IPPE given in the first year.

Straz found the AWV template. There is a preventive services template and an IPPE template as well.
