Need template or form for pain management visit

Looking for a form or template to use for pain medicine management visits for patients whom I handle the prescription of pain medication. Had Aetna into new office yesterday and they claimed I had to have a separate form/encounter episode for pain management and that we should not use the established patient office visit template for these visits. Does anyone have a form or template that you would be willing to share that you use for pain medication management visits.Dr. Beth Sullivan, DO
