If I may make a few suggestions...try to get it done arthroscopically (several small incisions are made for this). Also ask about a scalliene block. This will numb up the arm and shoulder for a good 12-24+ hours. Then inquire about the placement of a pain pump. When the block wears off this will have kicked in. It usually contains Marcaine or Lidocaine (surgeon dependent). The pain pump infuses at a slow rate to allow one of these numbing meds to go into the shoulder. Most facilities have them and/or can get them.
