Hope this helps you add these services to your clinics. and i found we have been doing this for free for the last 8 years. I print out a chart summary from my emr AND REVIEW THE PATIENTS PAST MEDICAL AND SURGICAL HISTORY, MEDICATIONS, ALLERGIES, CURRENT PROBLEM LIST, FAMILY HISTORY, SOCIAL HISTORY, PHYSICIANS AND OTHER DOCTORS, ADLS, DEPRESSION SCREEN, FALL RISK ASSESSMENT. I then print out the cms preventive services template and review it with the patient leaving open circles next to services they need, check marks next to those already provided, and x out those that don't apply. I then routinely check lipid, fasting glucose, BP, height, weight, BMP, +/- A1c in office if diabetic. I examine other systems if patient has complaint. I do a brief MMSE and document in psychological history with date performed. I add a line in past medical history that AWV completed 2012. I refill medications I add lab codes and e/m code with -25 modifier and enter the G0438 for the AWV or G0402 for the IPPE slightly more complex visit. See the preventive services template attatched for codes and coverag. I then type briefly what preventive services and preventive measures instituded (think Aspirin , or target LDL, or target A1c, or BP) and that a copy was provided to the patient. I usually complete this in about 40-60 minute visit. Next year should be easier for the next AWV will be different code. I would have summarized my workflow earlier, but have been busy with year end taxes. I really think these services will improve my patient's care and increase revenue per medicare patient by $100-150/year.
