Dear Doctor Seto.

I really apreciate yur email below.

I am a Family Doc that embarked with my wife Ardiana in the journey to open our IMP.

Ardiana and I never received any formal training in the health care business or how to open a practice, sound familiar?

We have so many questions and we have each other inside the boat when the water is ruff or wavy.

I found an oasis with this list server, because I can ask and will have many people going the extra mile to answered our questions since day 1.

I never find that in my community because here where I live primary care is very toxic with each other.

I contacted many primary care doctors around me and I am sincerely disappointed. I think there are patients for everybody so there is not reason to behave like that.

I just wanted to thank you for your intervention about this.

whenever we are at work and she ask something and I have no clue I tell her go ahead and ask in the list.

anyway I will leave like that, Adolfo

Steve, I think this post does make you sound callous, and I would guess it gives others on the listserv an unfavorable impression of you. There will always be someone on the listserv who asks a question that may seem obvious to you, but may not be obvious to everyone else. Since one of the main goals of this listserv is to help those who are starting up or have their own Ideal Medical Practice, we should all make this listserv as comfortable, collegial and safe a place as possible to ask the most basic, as well as the most difficult, questions.

Be respectful and considerate in your postings. Please try to remember there is another person at the other end of your message. Thanks for your cooperation.

hahahaha... adolfo.. youre too funny.

i welcome all questions even though i dont have the answers to them..

and steve too since he represents that aspect of me that's sick and tired of THE

business that i am still so in love with...

but tell you what...

when i came to my area, i was young and chirpy that a specialist told me, 'hey

missy... youre too young and chirpy... i bet you dont last too long here...

well, she had been sued by medicare and then medicaid for so-called fraud in

guesswhat overcharging'

so a dozen years later, im still young and chirpy, still learning the ropes of

staying in business and dealing with insurance and im now more successful than

her.

cynicism is what i fight everyday... and if i do become a cynic.. that would be

the day the shingle will change to a snowboard!!

Wow!

I truly did not mean to be callous and if anyone was offended, it was not meant

to be taken that way!

Maybe I get frustrated for all docs who keep getting their chains yanked by the

third parties, yet still go groveling back to them to yanked some more.

I know we all don't practice in areas where a third party free practice would

work, but the writing is on the wall that third parties are clamping down even

harder on private practices, so anyone who does not understand the rules will

get crushed.

I worked for 4 years as an employee in a two patient practice before going out

on my own, against the tide of what was occurring back then. I gave up third

parties other than Medicare in 2008, again bucking the tide.

But I always had a plan and a good understanding of the business aspect of an

office.

If I had to do it again now, in this healthcare climate, I don't know if I

would. Or I would open a concierge-lite office with low overhead, like an IMP.

So sorry if my frustration caused any ill feelings. It was not meant at anyone

directly, but more at how the system is so so stacked against private practice,

that those who don't understand the rules, are likely to get crushed.
