Guest guest Posted March 11, 2012 Report Share Posted March 11, 2012 Trip....Just some thoughts....School trip.....I have been to Russia 3x and definitely antibiotics, Epi with a note to carry this on the plane), albuterol if anyone is remotely allerig/asthmatic, antihistamines, cough drops, mucinex, cough suppressant, probiotics, topical antifungal cream, hydrocortisone. Dont forget parental permission slips to treat the kids and maybe make a list of standing orders and have the parent and their doctor sign....just in case. What medical conditions *do* the kids have?About palliative care.....I am not sure why you are so against a two tiered pay scale. If you want to be fair then you charge for the work for you do. If you do more work then the rate for that service should reflect that. I am not trying to start a debate, but we dont have socialized medicine yet where you will be poorly paid regardless of your work. I did a three hour presentation on socialized medicine/universal healthcare in grad school. I researched it, had a guest speaker who was visiting from England and I have been to Russia 3x. I also worked with a doctor from Poland, a nurse from Germany and know many Canadians.Therefore, I consider myself well informed and to be available 24/7 is a huge commitment. I would have patients sign up for the level of care they need and have a package you offer that suits their need. Just like a car wash...if you want the basics it is the economy level, and if you want a few more it is the deluxe and if want the works it is the Super duper and that is top notch. Cable TV is like this too. Basic cable all the way up to having it all along with internet and digital phone. You are not labeling patients, you are providing clear guidelines as to what you are able to do a and what it will cost. You may be able to say any new palliative patients will pay the new rate but existing patients wont have to because they are established patients. I dont know if that is "kosher"-so-to-speak but you have to do what is fair to you and doesnt burden the patients so much it is cost prohibitive.I dont accept insurance so there is more flexability but I am sure I will earn this way. I just cannot justify having them control the healthcare I provide. I am on my own to do what is best for my patients. I understand not everyone can do that or desire to and that is okay. I just feel I want a bit more latitude, too soon to know how it will go, but I am trying.Anyway, just some thoughts. I look forward to hearing how this will go for you and your patients.
