We are pleased to enclose a special invitation to the Hands-On instructional Workshop on Facial Aesthetics.

The goal of this innovative program is to provide education across a progressive learning continuum too increase knowledge, expertise, and confidence in non-surgical facial rejuvenation achieved by injection techniques.

HANDS-ON FACIAL AESTHETICS WORKSHOP, LEVEL II scheduled as follows:

Date/Time: Wednesday September 21, 2011 5PM – 9:30PM

Location: The Westin Seattle, Seattle, WA

Address: 1900 5th Avenue Seattle, WA 98101

Attendees will have the option to participate or observe during the hands-on training. If you are planning to participate as an INJECTOR, you must provide the following onsite:

One live consenting model (Please note, we can only accommodate one model per attendee). Your model should ideally arrive at the start of the workshop to allow for patient prep, or no later than 1 hour after the start of the workshop. NOTE: The final decision on treatment area for injection is based on expert faculty evaluation of model and student skill/experience level.

Injectables and Medical Supplies (see suggested types/quantities can be found here)

(You may use any FDA approved injectable products)

To participate as an injector, you must also provide us with the following Qualifying Documents:

A copy of your State Medical License (and Registration, if your state requires one).

If you are a PA, NP, RN or other nurse wishing to inject during the workshop, must provide us with a signed Acknowledgement from Supervisory Physician Form (download it HERE)

You may return the Qualifying Documents to us by either:

Bring copies with you to the workshop***

***Please note, if you do not provide us with these Qualifying Documents, you will not be able to inject at the Workshop; you will only be able to OBSERVE the Hands-On Session.

3) The online Pre-Workshop Survey you completed when you registered was developed to give you, the learner, and an opportunity to identify your most important areas of education to our Faculty. This survey allows the instructor(s) to tailor the curriculum to your learning goals. You may modify your responses to this survey by downloading the form HERE and faxing it back to us at , up to 48 hours before your workshop date.
