I am new to the group and feel very thankful to have been guided to it!

I am changing from a traditional hospital-owned FP practice to a solo Integrative Medicine practice providing consults and primary care in an IMP membership fee model. I'm looking to launch in a year. There is no one in my area doing anything like this, and I'm looking to build my network of people who are doing something similar!

I am located in NH, north of concord.

One of my biggest challenges is calculating how my practice will grow. I don't

think my current numbers are a good reflection, because the new practice is very

different.

How to estimate practice growth?

Good questions! For the consult part of the business I will be to providing an

integrative medicine approach for specific health issues brought by the patient.

Those issues are varied depending on the person, but can include fatigue,

prediabetes, cardiovascular risk reduction, athletic performance enhancement,

etc. My integrative medicine training provides me with a whole person approach

and expanded " tools " beyond conventional medicine including lifestyle

counseling, mind body approaches, use of supplements and botanicals when

appropriate (as examples). Referrals could come from any provider or the patient

may self-refer. There is a thriving Center for integrative medicine an hour or

so away, but I would be the first in our area and the first to combine

integrative primary care.

I'm very excited and think the this will fit within an IMP nicely.

Does anyone have experience changing a conventional primary care into a

membership model?

My history is probably documented in some old posts that you could search the list-serve on but brief summary below as fairly akin to your question.Started traditional hospital owned practice out of residency.  After 6 years of inability to find partners that had same goals as I and growing very intolerable of working for large hospital corporation, I started a solo-solo IMP integrative medicine primary care practice.  I share space with a gentleman that does chiro/acupuncture but we're essentially independent other than the shared website.

I had about 2000 total patients at the original office and probably 1200 active patients (using seen in prior 24 months as " active " ).  My financial model at the new practice was mixed insurance with additional Non Covered Benefits Fee (all office visits use traditional insurance and patients pay global yearly fee for all services outside of office visits).

Basically accepted first 500 transfers from old practice and then closed to any further patients.  If I would have allowed everyone that wanted to join it probably would have been around 750 patients.  Increased fee after 1st year with some additional loss resulting in current panel size of 400.  Basically now have only about 15-20 patient turnover per year which I fill from a waiting list.

Thus, not a completely membership based model but some patient financed component.  While I used some integrative techniques at my original office, the new office via its name and the philosophy I spelled out to patients clearly stressed the integrative component and has continued to grow in that realm quite a bit in my 3 years.
