I don't mean to sound cynical, but what you are saying goes against a lot of information that I have been given. For example, typical monitor frequency bandwidth is 0.5 to 40Hz while the diagnostic bandwidth is 0.05 to 150Hz. It is my understanding that the value of the upper end of that range is questionable, but that the majority of the value in a 12-lead is found in the lower end of that diagnostic range (0.05-0.5). I also know that I have seen 2 different ECG tracings from the same patient where the only difference was the frequency, and they were significantly different. I would also like to address the value of a prehospital 12-lead with short transport times. I have personally had patients that, after receiving a full ER work-up, the ONLY thing that sent them to the cath lab was my prehospital 12-lead. The patient's ED 12-lead was " normal " as was her blood work. She had also recently has a stress test that was also unremarkable. When she went to the cath lab, she had significant blockage in 3 arteries and was told that the blockage was potentially fatal. Our transport time with her was less than 4 minutes. Most people think that the only purpose for a prehospital 12-lead is early ED notification. While that is an important aspect, the value of a pre-treatment 12-lead can be immeasurable.
