Patients weren't happy about that after a few visits! (ignorance is not bliss!) To: Sent: Wednesday, February 29, 2012 1:32 PM Subject: Re: Medicare Deductible $140-160 Helen, I always bill it first because unless the patient really, truly knows what their secondary covers (I have found that few do), you end up collecting twice from the patient (which confuses them - "but I paid you when I saw you" or refunding because the 2a picks up the payment. I have found that most of our patients that show a copay on the secondary actually aren't required to pay it. Once I have billed it once for the current year, then I put a note on their demographics ("collect $20 copay for 2a at time of service") for future visits that year. If you want to collect on DOS, then I would have the patient fill out a credit card authorization and then charge them once you have received the EOB's back from insurance.
