If two people on the waiting list are equal in terms of blood type match, size and severity status, then how long you have been waiting determines who receives the offer. In addition, being on the waiting list alerts the transplant organizations and policy makers that there is a need. A notice of liver shortages is taken much more seriously if there are 16,000 waiting and 4,000 txs a year (4 yr backlog) than if 2,000 are waiting (6 month backlog). On the other hand, if you are not listed you don't have to think every call might be THE call. Your wait will be shorter. With status 2s making up less than 5% of the waiting list and receiving 40% of the transplants, national average wait once you are 2b is about 4 months vs 6.5 years for someone who stays status 3. So your docs philosophy is to wait off the list until you can get in the short line. Of course others who have been waiting in line will jump in ahead of you as they move from status 3 to 2b with their accumulated waiting time. So you might spend 8 months to a year at 2b, while someone who had waited at status 3 for 4 years might only spend a month once classified 2b.
