I followed up on some comments posted on this list a while back and made a

card to be right on Mom's front if she's ever in an ambulance or hospital.

I uploaded it to the file section. It is formatted to be printed on Avery

8386 postcards (4X6). I'm going to laminate mine and using a hole punch put

two holes on it and attach a large safety pin. In an emergency, it will be

in the " grab and go " kit.
