Guest guest Posted June 5, 2000 Report Share Posted June 5, 2000 The patient is in for regular check up with his main complaint being hearing loss. ASSESSMENT: 1. Hypertension, controlled. 2. Seizure disorder, stable. 3. CVA and mid left hemiparesis in 1992. 4. s/l Prostatedream (or prostateream) in 1992. 5. Hearing deficit. 6. Degenerative joint disease. This is the only mention of this in the report. I have tried to get prostate cancer or prostate carcinoma or even prostectomy out of this but is sounds like dream or ream run into prostate like it could easily be one word. Any ideas? Bev W.
