In preparation for my babies' birth last fall I drafted my own form

to sign declining the HepB vaccine and assorted newborn meds. I

thought I'd be proactive and wordprocess that document to be my

vaccine waiver at the pedi's office. My daughters are 7 mos. old

and the pedi hasn't given me a hard time (yet?); I believe he is

under the impression that we will follow a " catch up " schedule when

the girls turn 1. That is when I anticipate a potential conflict

and at a minimum, being asked to sign a waiver.

Does anyone have one that they'd be willing to share?

I welcome any and all feedback on what I have written.

Vaccination Waiver

I have had the opportunity to discuss the American Academy of

Pediatrics vaccination recommendations with my child's

pediatrician. I understand the following:

• The alleged purpose of and the alleged need for the

recommended vaccines.

• The risks and alleged benefits of the recommended vaccines.

• If my child does not receive the recommended vaccines, the

consequences may include:

& #61656; contracting the diseases the vaccines are alleged to prevent.

& #61656; allegedly transmitting these diseases to others.

& #61656; the alleged need for my child to stay out of daycare or

school during an outbreak of these diseases.

Many factors have been considered in reaching the decision that my

child's best interest will be served by declining the recommended

vaccines.

I know that I may re-address this issue with my child's healthcare

provider at any time and that I may change my mind and accept

vaccination for my child anytime in the future.

I acknowledge that I have read this document in its entirety and

fully understand it.
