I start patients on a small dose (5 mg PO t.i.d.), with a gradual dose increase of 5 mg every 4 days until 20 mg t.i.d. is reached. An alternative is to prescribe 10 mg PO t.i.d. and assess the efficacy and side effects before further dosing adjustment. Side effects such as nausea, sleepiness, or dizziness are fairly frequent and are unacceptable in some patients.
