Attention All Parents, Family or Caregivers for Someone with an ASD

We are conducting a research study in which we hope to shed light on how autism

affects you, the caregiver for someone with an autism spectrum disorder. It is

very easy for you to help too! Below are directions and a link to a short

questionnaire. If you choose to participate, simply read these instructions and

clink on the link where you will be asked a few short questions. Thank you for

your time and we very much appreciate anyone who chooses to complete the survey.

A team of researchers is seeking volunteers (18 years-of-age or older) to

complete an on-line survey pertaining to caregiving for someone with an Autism

Spectrum Disorder and its effect(s) on spirituality. Survey completion is easy

and typically takes less than 15 minutes. Should you wish to participate, please

read the following instructions first and then click on the link below into your

address bar to access the survey:

1. Once you begin taking the survey, it must be completed in a single sitting as

there is no provision for saving responses and returning to finish it at a later

time. Again, survey completion should take no more than 15 minutes. There is no

time limit for completion, however, so please take as long as you need to

provide honest, well-considered responses.

2. To navigate/progress through the survey simply click the " Next " button at the

bottom of each page. You must complete each question before going to the next

page. You can go back and change your responses at any point, however, before

submitting your survey.

3. When finished with the survey and after arriving at the final page (a thank

you page), click the " Done " button at the bottom of the page and then close out

of the survey.

4. Please complete the survey only once to ensure the integrity of the results.

5. To optimize viewing, please maximize window size if the survey does not open

in a full screen format. Because of minor compatibility problems with some

Internet browsers, the survey format and spacing may be disrupted on your

computer. Please be assured that this problem should not affect your ability to

take the survey and will not compromise your answers.
