Guest guest Posted December 17, 2010 Report Share Posted December 17, 2010 what does voriconazole do that less expensive antifungals like the ketaconazole oral my son is taking with no positive effects, 200 mg per day, has been on for a couple months, will do another week of this ineffective trial and then on to the next thing.... other than candida, no fungal infections have actually been cultured from his body fluids. sue v. I had on and off pain on the eardrum, eustachain tubes, etc for nearly 3 >years. Drs thought I may have to get tubes in my eardrums to release the >pressure (and get a culture). Guest guest Posted December 17, 2010 Report Share Posted December 17, 2010 the spectrum of action of ketaconazole and fluconazole are pretty limited. Itraconazole has a better spectrum, but caps have little bioavailibility. Itraconazole oral solution is good. Still many fungi are resistant to these, so just like antibiotics one uses 2nd generation triazole like voriconazole or posaconazole. Amphotericin B was the drug of choice for systemic infection, but is more toxic on the liver and for most species does not beat vorico. Another category worth a try are the echinocandins like caspofungin. Actually vorico and caspofungin can be combined for infection that have reached the CNS. Testing is extremely primitive - unless your son is within imminent death from sepsis, the vast majority of doctors wont consider a systemic fungal infection other than candida-->fluconazole or something like that. You have to find a dr that will let you try what you want, or order from overseas (which I regularly do).
