Subject: To: "dianaotwell@..." Cc: "Kim Duong" Date: Saturday, April 14, 2012, 8:40 PMHi ,I hope you and your family have been well!Kim Duong mentioned you might be interested in participating in my research study through San State. Thank you for your interest! I am researching Levels of Stress in Parents of Children Diagnosed with Autism and Down Syndrome.Your participation will involve completing a short Parental Stress Scale Survey. This survey is comprised of 18 statements

that describe feelings and perceptions about the experiences of being a parent. It is anticipated that this will take you no longer than 5 minutes total to fill out and return to me.I will also be collecting data from your responses to the Ages & Stages Questionnaire – Social Emotional (ASQ-SE) document that you filled out upon entry into San s Regional Center’s Early Start Program. In order for me to access the ASQ, I will need your signature on the Parental Consent Form, also attached.Your name and your child's name will not be used in the study. All documents are kept confidential. Please fill in the parental stress scale and sign the consent form. You can email both back to me at this address.Your participation is greatly appreciated! Thanks so much, ! HartEarly Start Service CoordinatorSan s Regional Centersahart@...
