I have a chronological listing of the major medical events like

1981 Spinal Infection

1983 Car accident c-2 fracture

1987 Discharge from military with DJD

etc

and then I list problems since last appt like

22 days of trapped ulnar nerve

- took methylprednisone pack, amatriptiline, Tylenol 3, and flexeril with no

reduction in pain

Sorry last one got sent before I got finished. Also, if I have had any labs or

xrays done since the last visit I always take a copy of the medical report.

Anytime I see a different dr, I always have them send the rheumy all their

notes. Example: I recently had to see an oncologist and had a lymph node

biopsy and a ton of blood work. I took ALL the results with me when I went. I

always ask for a copy of any medical work done so that I always have it.
