It would seem to me that RA will find a way to escape into the fog and elude it's pursuers until there is a screening test that is simple and available to all. If early intervention holds the key to remission there must be a " gold standard " test that would detect the disease while it is asymptomatic in every cohort. The much heralded PSA test for Prostate Cancer is beginning to lose advocates because they say too many are being treated who would not have had disease progression that was life threatening. Others say that PCa deaths are declining because of the screening of men at a younger age. Would screening for RA start at birth? Or 5 years? It would seem that early intervention would need to be just that. The " window of opportunity " in RA is still open after people begin to show symptoms of RA. Or so it is believed. The emphasis is on getting an early, accurate diagnosis and then aggressive treatment before the window closes. Due to the heterogeneous nature of RA, I think it might be difficult to come up with one, and only one, marker. Not an MD I'll tell you where to go!
