The majority of docs tell their patients to take their folic acid

every day.

Hi Peggy,

my Rheummy says Absolutely no way take the folic acid on the day of the mtx, so

did my last rheummy, but from what I believe they all have different views on

this...check with yours.

With all the crap I listened to with my former Rummy, I'd investigate before

I made any changes. First, I was told that since I took my MTX on Sunday, I

had to take my blood test on Thursday, 4 days after the shot to let the

blood settle and the test more accurate. Then, I was told that I had to do

the blood test on Tuesday because they weren't in the clinic on Thursday and

Friday. Also, there was no mention of Folic Acid, which was causing me

problems. I finally found out the real facts here and adjusted my FA to work

right. Whatever you do, do research as much as possible, and listen to the

knowledgeable ones on this list! I take my FA every morning and don't stop

on any day for any reason.

actually my pharmacist also agreed that folic acid and/or my vitamin

b complex should *not* be taken on the same day as the mtx.

i'm sorry, i think he did but at that point i was so overwhelmed, i

don't remember. when i go for my refills i'll try to remember to ask

and actually retain that memory till i get back online again ! ! !

hello, i'm taking folic everyday w/prednisone and mtx once a wk. now i'm

beginning to wonder. why wouldn't taking it with mtx be o.k.? does anyone know?

Kathie, just do what your physician told you to do. This is a controversial

issue. Even how much folic acid to take isn't agreed upon (but it's usually

from 1 mg to 5 mg daily).

The physicians and researchers who advise patients NOT to take folic acid on

the same day that they take MTX are worried that the folic acid will

interfere with the effectiveness of the MTX. In the other camp, there are

those physicians and researchers who don't believe folic acid will

significantly interfere with the action of MTX.

The problem is that we don't know exactly how MTX works in RA. Lots of

clues, but no definite answer yet.

From what I've read, the position that patients on MTX should take folic

acid every day has the edge.

From what I've read lately, it's a guessing game as to what to take and what

not to take. Physician preferences are a toss-up, too. I think I like my

situation, I can pretty much dictate my own med schedule and the Rheumy goes

along with me as long as I can explain my reasoning. Take what you can/want

to and if the doc complains, change it to what you and the doc can agree on.

It's a heck of a way to run a railroad!

As some additional information on the discussion, I recently changed

docs. The original doc recommended NOT taking the folic acid on the

day I take the mtx, and the new doc DOES recommend taking it. My

dosage was recently increased by the new doc to 20 mgs. mtx. and I

feel great! I think the folic acid helps.

From what I have read here and elsewhere, and from talking to both

my rheumatologist and the pharmacist, this is my take on the issue.

Folic acid and folinic acid (Leucovorin) both reduce the

effectiveness of Mtx somewhat. HOWEVER, they are also very good at

reducing the side effects. If you can tolerate the Mtx without

taking large amounts of folic or folinic acid, particularly on the

day you take the mtx, the mtx has a better chance of being

effective. But if the side effects are bad enough that they are

likely to lead to you choosing to discontinue the mtx, it's still

better to take the mtx and leucovorin on the same day than to stop

taking the mtx altogether.

In my case, I just get too sick form the mtx to be able to tolerate

it without the leucovorin... even with it, it's really hard to wait

the 12 hours my doc suggests before taking the leucovorin.
