Have you demanded that she give you a referral as opposed to asking for one? I need referrals for my insurance plan too and luckily so far the ped refers us wherever I want, but I would be very upset if she refused to send us somewhere. The other thing is if you are doing everything assuming this is apraxia, will a diagnosis at this point make a difference in the therapy your SLP is providing? Guest guest Posted July 31, 2007 Report Share Posted July 31, 2007 A. You need a new pediatrician. If you have made more than one to two attempts to get this referral then get a new doctor. If they are stating that your child can only receive a service/diagnosis from a certain place and then won't refer you, that is inappropriate. You also need to notify your insurance company, in writing, of this problem with the pediatrician. Keep all communication business like and unemotional. You'll get farther that way. B. Print out the ASHA statement on apraxia and show it to your speech therapist. This should give the slp the info to let her know that she can diagnose. That said, many folks (i.e. insurance company, school system, etc) will give your diagnosis a bit more authority if it comes from a physician. So, an eval with a developmental ped and/or a pediatric neurologist would probably be a smart move. But first.... new pediatrician. Start making calls tomorrow to take positive steps toward getting your child the care he/she needs. You've been given the run around and it sounds like the time has come so.... you need to move in a new direction. I have had 2 and they are soooo different, the second who is willing to do any test and refer out. Also, I wonder if self-pay option would get you through the door! If there are no major health concerns that require long knowledge of your child...I would suggest a new pediatrician. If your doc won't refer to a developmental ped...a child in speech therapy - maybe you need a new pediatrician. Guest guest Posted August 1, 2007 Report Share Posted August 1, 2007 Wow - what a mess.What is going on with your pediatrician when she says you need to see a dev ped, but then won't give the referral? First step, get a new pediatrician. I know it's hard, I had to do it too. But do it - and set up an interview with the doc before you sign up with the new pediatrician. They do have those appts - but if they refuse see if you can get a phone interview (due to your child's special needs and needing to see if they can be met at their practice). I don't know how old your son is - but look up the developmental language milestones for that age and compare to your sons level. My son has been non-verbal since forever. So we just say - he doesn't speak, there is an obvious problem. As we say, we are fortunate his issues are so severe b/c then there is never a question as to " if " we get a service or referral. You may have to make a case by saying " typically a three year old has x words and is speaking in x word sentences and my son only uses single words " or whatever. You may be able to even give him a label like " he's speaking at an 18 month level and he's three - there is a problem " . I'm just generalizing. B/c my son didn't have any words we typically heard 'we can't diagnosis apraxia/dyspraxia until we start seeing speech errors he's making'. So we've never had an apraxia diagnosis. We work with the speech therapist to treat the symptoms - working on a communication system that takes the focus off of speech (which caused him to clam up); worked on waking up his mouth with toys, etc. (which he was reluctant to) and getting all the speech therapy we could out of the school (again our issues are severe and it makes it easier). BTW-he started using words Oct 2007 at age 5 and a half. It's hard but don't give up. What type of waiting list does the apraxia clinic have? Can you make an appt while you find a new doc to get the referral - some places have waiting lists that are several months long. You could be working on the new ped and the referral to the dev ped and the clinic while you wait for you appt time to come up. Many places will make the appt but just want you to walk in that door the day of the appt with the referral. Just some ideas. Guest guest Posted August 3, 2007 Report Share Posted August 3, 2007 I have contacted the dev ped office at Cincinnati Childrens and it looks like a 6-8mth waiting list and they require a referrel prior to getting on the waiting list. To transfer his speech and OT is at least a 3-8 mth waiting list. His speech therapist told me today that she believes he is apraxic and NOW she says she can diagnose. However, she won't because she states hes too young. That she will continue to watch him over the next 6mths. I am trying to transfer care to somewhere closer, so not sure if I will be there in 6 mths. She said that it wouldn't matter if she diagnosed him anyway, because it wouldn't change his care or how she teaches him. She basically gives him toys to play with and then takes it away and grabs his hands to make him do " more " sign; also blows bubbles at him and repeatedly says bubbles over and over and then takes them from him. She also basically chokes him with one of the z-vibes. I don't see him getting anything out of therapy, except anger and tears. And then call them once a month or twice a month to confirm where he is on the list and " thank them for their time " . Why? Squeaky wheel. And remember.... a 6 to 8 month waiting list in reality is often a 4 month waiting list. Especially if you are the kind sweet lady who calls to check every now and then. You've got to take this one step at a time. So... step one is getting that referral. And in your case, that may mean a new pediatrician. Tell me this....are you on an HMO? if so, this is maybe why the pediatrician isn't referring. Any services he permits affects his bottom line. You may need to go in with an armload of articles, ASHA statements etc and demand (politely of course) your referral. Try this and see what happens. Informed parents who know their child's diagnosis, come prepared with credible evidence and know exactly what is needed tend to get a bit farther. Afterall.... if you're that well informed he will instantly realize that it may be a liability to not give you a referral. The other method you can try is just to tell him you can't sleep and you need to calm your worries and have him seen. If you're on an HMO however and his motive is money, this won't work. In order to know what that help is I would read all I could, including the late talker. I would also consider NACD, even if the evaluation is done by video due to distance and $. In some ways, your therapist is right...label or no label, the right therapy helps. The problem is it does not sound like that is what he is getting and that too young nonsense is nuts. Best Wishes!
