I've got the same problem with my rheumy --doesn't believe the spine is directly affected by RA. Yes, I decided, based on my doctor's ignorance of this and ignorance of other issues, to find another rheumatologist. I'm hoping I can find someone good who is in-network on my insurance plan. My benefits aren't great, but the out-of-network costs are out-of-sight!
