If she is " covered by the state " , then I would refer them to her Medicaid number.  Often-times, I have to do that with my younger son.  We have primary insurance with Medicaid secondary, and they often " forget " to run it when we go to the hospital.  Once they do that, your problems might be over.  

The only thing I have NOT had Medicaid pay for was an antibiotic from an MD (pediatrician) once, and a certain brand of stimulant meds.Good luck, and I am terribly sorry for your daughter's suffering.  I am thankful she is okay.
