We know that children with developmental disabilities sometimes require an integrated array of services, often a financial strain on the family budget. Sometimes children do not receive the services they need that could benefit their life now and into adulthood. Our vision is to close the gap in funding for clinical treatments. We are very excited to offer a small number of scholarships in June 2012. You may visit our website for forms and instructions at www.innovativehealthfoundation.org. How to apply: Complete the parent portion of the Supplemental Clinical Scholarship Application. Complete and sign the Authorization to Release Protected Health Information.Provide the application and signed release to your service provider.Your service provider will complete treatment sections and the recommended treatment for this scholarship.Your provider submits the application to IHF via email or U.S. Postal mail.If your service provider is not a member of the IHF provider network, then they will also need to complete the Provider Application and submit along with your scholarship application. WE NEED YOUR HELP Our foundation is producing an informational video to tell the stories of families with children with developmental disabilities and to demonstrate the financial need in our communities. We are looking for families willing to be interviewed and share the miracles and the challenges. What are we asking from you? Your consent and agreemeet to take videotape and digital recordings of you and your children to be used in print and electronic form publicly and privately.Meet us at Medlock Bridge Neighborhood Playground in s Creek, GA on Saturday, May 12th; times determined based on your schedule Film of you playing with your child and interacting on the playgroundFilm one-on-one interview with parents; sample list of questions belowApproximate time commitment 30 to 45 minutes Sample Interview questions: How do you take care of your child? Tell me something about your child’s present condition.How old were they when they were diagnosed? What was your experience working with practitioners such as your pediatrician or therapy providers? What changes have been made in school since he/she was diagnosed? Are there special services for him/her? Are you satisfied with your school’s program and services?What have you learned about raising a child with a developmental disability?What advice can you give to other parents raising a child?
