The attached is the new form and procedure being used in IEP meetings to determine OT and PT. Many parents are just being shown partial pages. This is the complete document. Page 4 is particularly useful as it gives the definitions and frequencies for the types of service. If schools are going to use this form, we might as well have ALL the pages and be FULLY informed of the options. I encourage you to fill this out before the meeting in order to truly think about which ratings should be considered for an individual child and ensure that the child receives the needed services at the appropriate frequency.
