Tim,

Last time I researched this, there was no procedure code for

just removing staples or stitches. So I use the time appropriate E &

M code and the ICD-9 that is “Attn to sutures..”, which is a v code

and I can’t remember it at this second. There are now codes that

are just wound and dressing checks vs. suture/staple removal.

there isn't one If you put the staples or stitches it in is included that you take them out no chargeif you did not put htem in use some e and M code like 99211!!

There is no code for suture/staple removal. It is included in the global fee for the procedure. If they go back to the ER, the ER has to take them out 'for free'. I've just in the past billed it as 99212 and it gets paid. Use 'wound' (sometimes cellulitis if it looks red- this gets it out of the global fee issue), as the ICD. Usually paid but sometimes booted out because they say it's within the global fee period. The code is V58.32 " Encounter For Removal Of Sutures; Encounter for

removal of staples " .

And for clarification, if you placed the sutures/staples yourself, you

would not be allowed to bill for their removal as that is considered

part of the global charge for putting them in. However, if somebody

else put them in, then you are allowed to code it with the appropriate

E/M level code.

, the V code can go with an E & M for the visit, but do

you know a CPT for the actual removal of the sutures/staples? The only

one I have ever found is only for if you have to do it under anesthesia (15851).

I once had to remove over 60 sutures that had been placed in the

ER…I tried REALLY hard to find a code for that, but never could.

The AMA has clarified billing for suture removal as it pertains topostoperative management in the Spring 1992 issue of *The CPT Assistant*:"Q. What code do I use to report suture removal?"A. A quick answer would be- there is no code for suture removal.However, a better answer would be: If the suture removal is performed by thesurgeon during the global period after the surgery, the suture removalwould not be reported separately, since this is included as part of thenormal, uncomplicated follow up care related to the surgery. My coding book says that that code is for the same surgeon, but

UNDER ANESTHESIA, OTHER THAN LOCAL . Then 12851 is for the same only “other

surgeon”. I don’t know about you, but no general anesthesia in my office,

just my clever patter to distract..;->

I usually bill a 99212, if I did not put them in. As stated, you can't bill to

take them out if you billed to put them in.

Oh, I would never use a 99211.Basically, my asking the patient a few relevant

questions and taking a few minutes to take out staples/sutures and checking the

wound is nothing less than a 99212.Thanks folks for the info on the

staples... should have figured it was the same as sutures, but I wondered since it

is a bit more work in a way, and those staple removal gadgets do cost money. I've

had some for 2 yrs and not had a chance to use them. After this, it's getting

cleaned and autoclaved from now on. I'm not going to buy more. Actually, at

the IMP Camp this June, maybe I'll raffle off some of them... and send the money to

yeah, that must be new in the last year or two. My sources don't have anything

beyond v58.3 and it includes change of dressings and removal of sutures.TimOn Tue, March 25, 2008 10:43
