You can check their policy or work something else out with them. If you are opting out of medicare in your own practice and need to opt in in your moon lighting, then you need 2 NPI's. I have one individual one and one corporate one ( apply for NPI as a group practice, type 2). If you are not incorporated, I used smallbiz.com to be incorp. Does any one know the details or where to look for specifics of the rules behind opting out of Medicare and then having to file an affidavit every 2 years? What happens if a physician does not file the affidavit at 2 years? It is my understanding that you are " opted back in " if you do not file the affidavit.

When exactly do we as physicians become irreversibly tied to Medicare? Was it when we received our M.D. degrees? Is this linked to our licensure? Our DEA number? I really do not get it.

When I retire, do I need to continue to file affidavits? Is this " law " tied to billing anyone for medical care? Where does it end and where are these rules located?

I really am confused as to the legality of this requirement.

Transition week #3 from traditional-take-all-insurance-companies to ending all insurance contracts and opting out of Medicare August 1. I am tired, but so far so good. Looks like about 50/50 split so far in patients leaving my practice verses signing up. If it keeps going this way, it will be perfect. I really do only want 600 patients, not my current 1200. Could meet current overhead with 400. Lots of interest from other family physicians in town. Specialists very supportive so far. Patients seem to understand after I explain my reasoning and patient care driven model.
