I'm a little confused on these prefilled epi syringes.

The epipen says...

But a lot of these other prefilled syringes say 1:10,000.

But these are all 1:10,000 -- which is the IV concentration, yes?

I searched and searched, but could not find a prefilled 1:1,000 epinephrine -- other than the epipen.

Anyone else with luck?

The non-prefilled are pretty cheap at 76 cents per vial -- but you would have to draw up the correct dosage -- but still can't beat the price.

Are you sure it's the correct concentration premixed?

If you are giving an IV preparation, IM -- it could be the 1:10,000 concentration you are giving IM -- and therefore underdosing.

I'm sure you've checked that, but was just curious.

–Absorption and subsequent achievement of maximum plasma concentration after subcutaneous administration is slower and may be significantly delayed with shock.10,11 Thus, intramuscular (IM) administration is favored.

Administer epinephrine by IM injection early to all patients with signs of a systemic reaction, especially hypotension, airway swelling, or definite difficulty breathing.

Use an IM dose of 0.3 to 0.5 mg (1:1000) repeated every 15 to 20 minutes if there is no clinical improvement.

–Administer IV epinephrine if anaphylaxis appears to be severe with immediate life-threatening manifestations.12

Use epinephrine (1:10 000) 0.1 mg IV slowly over 5 minutes. Epinephrine may be diluted to a 1:10 000 solution before infusion.

An IV infusion at rates of 1 to 4 µg/min may prevent the need to repeat epinephrine injections frequently.13

the previously mentioned epi in the screw together injector is actually only $4.20, not $7.00.

it states it is for IV use, however, i would be injecting it sub-Q.

much, much cheaper than an EpiPen.
