Also of note, the form has been updated. http://wwwn.cdc.gov/travel/contentIntCertOfVaccination.aspx AnnouncementRevised International Certificate of Vaccination or Prophylaxis Now AvailableThis information is current as of today, February 08, 2008 at 20:58 Updated: January 09, 2008 To comply with the International Health Regulations (2005), the Centers for Disease Control and Prevention (CDC) has released a new International Certificate of Vaccination or Prophylaxis (ICVP). All yellow fever vaccines given on or after December 15, 2007, should be recorded on an ICVP. The ICVP replaces all versions of the International Certificate of Vaccination (ICV) that are currently in circulation. CDC requests that all health care providers destroy any unused old certificates and begin using the new certificate. Currently, yellow fever is the only disease under the International Health Regulations (2005) for which proof of vaccination may be required for travelers. Previously issued certificates remain acceptable proof of vaccination against yellow fever as long as the certificate is valid. The new certificates are available for purchase from the Government Printing Office bookstore. To order, please visit http://bookstore.gpo.gov/collections/vaccination.jsp or call toll-free . Packages of 25 are available for $23, and packages of 100 are available for $51. Orders may be placed now and will begin shipping around December 1, 2007. The World Health Organization (WHO) in Geneva, Switzerland, also sells a version of the ICVP.
