Most of what is included can be found in other files/notes (like the IEP) but it doesn't hurt for teachers to be reminded, especially new ones that might not be familiar with special needs and have never dealt with an IEP before. And as your child grows, so does the massive school file for him/her! August 8, 2008 Dear Staff of ___________ Elementary School; My son, __________, has several food allergies causing him to be on a specialized diet and is only allowed to eat food that is provided for him by his family, unless prior permission is given. During special occasions (i.e. class parties) I will be happy to provide a few suitable treats that he will be able to share with his classmates. Food is not to be given/withheld as reward/punishment. It is also important that ________ drink plenty of water to stay hydrated. He will bring bottled water every day which can be refilled with tap water. It is necessary that he has access to water after physical activity. ________ is also very sensitive to environmental chemicals. Please be mindful of using cleaning products, perfume, etc in his immediate area. These may cause headaches and/or behavioral upsets as they affect his central nervous system. He will be riding a special needs bus (#________). He'll need a paraprofessional to meet him at the bus in the morning and bring him to the front of the school each afternoon. Since _______ still has communication troubles he needs guidance in recording the assignments/homework so I can be sure to do the correct work with him each day. Also, as noted in his I.E.P., he needs a daily behavior journal sent home. It is important for me to hear the issues (both positive and negative) so I can reinforce them each evening. Please keep a copy of this note in ________'s file in the office, as well as in his current classroom. Thank you for working with him. I look forward to him making great strides this year!
