I wasn't sure how to title this. My son has been a patient of Dr. G for almost 4 years (and doing great!). My son was a healthy, easy-dream baby until he was 18 months old. His tipping point seemed to be a flu vaccination he received the same week he got rotovirus. He was very sick all the time after that. His twin sister, OTH, was medically fragile from birth (they were born 14 weeks prematurely) . She had trouble breathing, and a lot of trouble eating, and she's still tiny (7.5 years old and 36 pounds). We learned when she was 26 months that food intolerances were making her so sick, and once we figured those out, she started to thrive and has been pretty healthy since they turned 4. That said, since November, she has had 5 episodes where she complained of sharp tummy pain, which then turned into vomiting. Three times, she was diagnosed with strep and took antibiotics. It happened again today - she woke us at 4 am complaining of pain in her tummy (right around her belly-button towards the right side), started vomiting an hour later. She threw up four times. I took her to urgent care because she was still whimpering that her tummy really hurt. They are supposed to start 2nd grade on Monday, so I was hoping it was strep again and we could start an antibiotic and still start school on Monday. As usual, the rapid test was negative, so they sent it to the lab and it's not due back until Monday. We came home and her tummy still hurt badly this afternoon. Urgent care recommended Tylenol, which did nothing for the pain. At 4 pm, I called the pediatrician (urgent care said to call her ped if she seemed to be getting worse) and they recommended Motrin. She has been asleep for almost 3 hours now, following the Motrin. Only once of the 5 episodes did she get diarrhea. She typically has mild constipation, despite a diet high in fiber/fruits/ veggies. Any ideas??? The ped wants to follow up this week. He wants to test her for strep when she is not sick. I did some research online this afternoon and it sounds a lot like abdominal migraines, but no one on either side of my family or my husband's has migraines. Does this sound like PANDAS? Guest guest Posted August 30, 2009 Report Share Posted August 30, 2009 Hi Donna & Sheri, Thanks for your responses. I think appendicitis has been ruled out, since the pain subsides with Motrin. The ped said Motrin would not be able to mask appendicitis pain, and also, this has occurred 5 times since last November. I think, too, that it's not a food intolerance. We've had tons of experience with food intolerances and allergies (an intolerance to corn was literally killing her as a toddler), and this is different since the episodes are so spaced out (a few months in between). Guest guest Posted August 31, 2009 Report Share Posted August 31, 2009 Kristy, This is probably way off base, but two thoughts came to mind when you mentioned pain near her belly button. One of my friends has two daughters whose diabetes first manifested as pain behind their belly buttons. Second, appendicitis can manifest as behind the belly button. I just thought I¹d throw those two thoughts in just in case... hopefully it is something that will clear up quickly on it own or with minimal treatment. The pain location she is pointing to doesn't seem to be exactly right, but the odd thing is the pain can radiate and feel like it's coming from somewhere else. Sometimes it seems to be in my back. Anyway, there are other 'itisis' in the gi area that just might respond to benedryl... The easiest way to tell is to give her a small amount - 1/4 tsp to half tsp should do it - of liquid dye-free benedryl and see if the pain reduces over a 20 - 30 minute period. It won't typically make all the pain resolve, but you would see a big big improvement if it is that. The pain is typically very sharp - I usually wake up thinking I just got stabbed or that my stomach is rupturing, and you vomit during the more acute attacks. Even after it improves, there is still a dull sometimes throbbing ache that just radiates and makes you miserable. It probably isn't gastritis, since you've made a connection to other illnesses being associated with it to, but it is very easy to rule out and it just sounds too familiar to me. I've had sooooo many 'attacks' after consuming fatty foods. I'm not sure if it's my gallbladder or gastritis, but I do know I will never eat fried food, fatty food, or full-fat dairy ever, ever, ever again. I have not had a problem for a few years since making the connection to fatty foods. Extensive testing (ultrasound, upper GI, etc.) revealed nothing. Guest guest Posted September 2, 2009 Report Share Posted September 2, 2009 Hey Kristy, With gallbladder issues (the fatty & fried foods are the clue there), there are plenty of times that inflammation in that area - " itis " wouldn't necessarily show up. There are different kinds of gastritis too ... and esophagitis, and sometimes it just feels like heartburn. For me, many kinds of foods - especially foods w/color red - artificial or natural - will set it off. I haven't had a scope so the diagnosis isn't firm, but it's most likely allergic gastritis. An x-ray did confirm gastritis, but a scope would help to find out if it is eosiniphilic or not. With the 'heartburn' feeling esophagitis, Zantac and such don't touch it much, but benedryl liquid - only the tiniest amount so as to not have the side effects - will knock it out. It's a pretty harmless test for many of our tummy ails. If you had a reactive gallbladder that was inflammatory, it would be worth trying the benedryl on it too. Benedryl isn't the typical treatment for gastritis, but all of the people I've met that have it, I've suggested it to them, and they tell me later that it was the first thing that ever helped. They tend to go to the stomach meds instead of allergy meds. Dr gave me Allegra, and that really rocks - after a few days of taking the child-size dose (I can't tolerate the full dose), the symptoms will go away completely, whereas benedryl I have to take 3 times a day and don't always get complete resolution. Allegra will actually fend off an attack, although I still try to remember to avoid the triggers. It only takes me a couple of months of no pain to forget I have a problem.
