I went from W+2 to one half W+1 (for example if it called for 5 lines twice daily, I applied 2 in the am and 3 in the pm). I also cut the Progesterone to one line(20mg) and cycled up to 5 lines(100mg) and back down to one line on day 14 to 28 for Month 1 and 2. On month 3, I just did one line on Day 14 to 28 and I am on Day 22 of that month now. I saw an endocrinologist in my area this week. The endocrinologist was great and she listened and took time with me. She does not think saliva measurements are accurate at all. She stated her concern was that it is impossible to know how much absorption one is getting with the transdermal application of creams. She did not want to use any compounded product as there is no testing or regulations associated with these products and hence no standardization. We opted to use Prometrium 100mg day 14 to 28 or Day 1 to 28. She left that as my choice..the only difference being period or no period. She also prescribed Divigel (an estradiol gel) to use daily at the highest dose which is 1mg. This is applied to the thigh daily and as you all have read, gels have better absorption than creams. Basically, she felt more scientific research needs to be done and agrees that the literature is woefully inadequate. I feel happy with this and will let you all know how I feel when I start the Divigel. My lowest Estrogen on my modified Wiley was 3.0mg and the highest was 9.0mg so it will be interesting, but I'm hoping the better absorption of the gel will make up for the difference in dosage amounts. She was also clear that you have to use Progestrone in order to prevent endometrial hyperplasia that occurs with the use of unopposed estrogen to avoid the cancer risk. Good luck to all.
