And his expertise in the advantages and problems of solo/small medical practice, is exactly what?

Here is the link where you can read all of the witness testimonies:http://smallbusiness.house.gov/Calendar/EventSingle.aspx?EventID=302953

3 physician testimonies and the one from Merritt Hawkins.  I just read through all of the testimonies this morning.  Not much new - fix SGR, doctors can't afford to be in private practice any more, etc.  One new item that I hadn't heard (or thought of) before was the reduction in tax revenue when a practice transitions from being physician-owned (and having to pay income tax) to being owned by a non-profit hospital, and thus not having to pay income tax.

Indeed. Where were the internists/ FP/peds/primary care people? Such late notice. Maybe we were not included in the hearing because we are invisible? Invitations lost in the mail? True, the AOA seems to be pro primary care. Thank goodness. Interesting. Are they also somehow being shut out of or competing mightily with the big hospital / mega center mill machine? you're right, the guy from Merritt Hawkins does have a handle on quite a bit of the situation, including what positions the pcps in the marketplace are and are not interested in. As he says, "In short, virtually no one wants to be Marcus Welby anymore." (My italics.)But I say, when people are sick most want a 'Marcus Welby' who knows them and cares about them. Me too.

I figured it was politics that they held  this testimony to say they listened s'okI am not sure a why  ACA or ARRA or whatever it is will ruin small pracitces?

Wasson doesn; t   think so. That man is so optimist it is depressing :)   But on the other hand he sees a  big picture and talks to CMS and so on.... it is about  how we all work together  , not who we work FOR. Of course  we have few ways to work together and  no  one really is gathering us up   noticing...looking for all possible PCPs..I am left out of or excluded  deliberaltey from so many things--well except the ones people ask ME to pay for.

Some organizational work is needed. To gather us up  The state medical association and state FP acedmy are not taking this onI admit to feeling fairly frantic this week in particular with  things swirling around about NCQA PCMH and Cigna and Aetna and ACOs .. and...

every state is different ....who to talk to??

it is taking me alot of energy I  sure do not have to stay on top of what is going and and try to be visibleIs this true for others?I am supposed to be a cool PCP doing  good work but instead my existence is threatened  ugh

Ideas?

Group,I have it on good authority that the AAFP was invited to the meeting but the representative had to cancel at the last minute. The AAFP will be submitting its thoughts to the Congressional committee shortly.

JEAN, WHAT I READ IN THE TESTIMONY, WAS THEY CANNOT MAKE IT ON THERE OWN. THEY ARE PAID TOO LITTLE AND WE MUST HELP THEM WITH OUR BUSINESS SKILLS. THEY CAN WORK FOR US AN WE WILL MAKE IT BETTER WITH ADMINISTRATION, IT, AND BUSINESS "SNAKE OIL!" WHAT WE NEED TO DO IS TO AGGREGATE THOSE OF US LEFT, AND FORM A COOPERATIVE. BUT HOW??? AND WHEN?? I JUST READ UNTITED HEALTH'S QUARTERLY REPORT. THEY PAID APOUT 81% FOR MEDICAL CARE OF SOME 27 BILLION REVENUE FOR THE QUARTER. ON 35 MILLION COVERED LIVES. THAT IS $771 DOLLARS/MEMBER OVER

THREE MONTHS IN PREMIUMS. $624/MEMBER IN MEDICAL COSTS. $147/MEMBER AS OPERATING COSTS TO ADMINISTRATE THE INSURANCE PAYMENTS OR ABOUT $1.63/DAY FOR ADMINISTRATION. I AM SURE THE $250 PER YEAR I COLLECT PER PATIENT ROUGHLY REPRESENTS $ 0.22 PER DAY FROM THE PATIENT, $0.22 PER DAY FROM MEDICARE, AND $0.22 FROM PRIVATE PAYORS LIKE UNITED. BUSINESS PROFESSIONALS WOULD NOT CONTEMPLATE THIS DEAL. UNITED COLLECTS $3,000,000 A YEAR PER 1000 COVERED LIVES. CAN YOU IMAGINE WHAT KIND OF HEALTHCARE WE COULD PROVIDE FOR OUR PATIENTS. WE NEED TO FORM A COOPERATIVE AND CUT OUT THE INSURANCE. WE NEED TO DO THIS IN EVERY TOWN FROM COAST TO COAST. THEN WE NEED TO TAKE OVER THE HOSPITALS

TOO. ANYONE WANT TO GET STARTED? WISHING I WAS KING FOR THE DAY! KEVIN To: Sent:
