What is the reason for the denial? I used to send my notes but don't need to any more. I always make certain I document actual time spent face to face : 10 am to 12pm for example. We have recently been seeing an increase in the denial of payment for Prolonged Service code 99354. Does anyone have words of wisdom or a template that they use that seems to help in the payment of this code?
