I have the same insurance and I just appealed it and appealed it over and over

and finally they paid..

I was attending a CME ( continuous Medical Education) for my license in health

care, the last speaker was a rep from Blue Cross Blue shield he said to all the

doctors, there is a golden rule, if the person appeals it 15 times go ahead and

pay it...

I was shocked but that is what I have done and it works.

Could not agree more, appeal it, get the name of the case worker and keep

calling them.  Health insurers want you to go away.  I was previously in

claims for Property and Casualty and we had an acronym; DDD.  Deny, Delay and

Defer.
