We are in Indiana. After reading Dr. G's book, I have been thinking of switching him to the protocol in the future. My only hesitation is that he definitely showed signs of distress/ASD right from the beginning (no obvious regression at any point due to some type of infection). We've only done the dietary (mainly SCD, but with little to no nuts) and supplement portion of the DAN protocol, and he is doing excellent (no behavioral problems and in regular class without support and friends!). However, he has a tic and persistant perservations (maybe OCD) that I believe are immune related (like PANDAS or PITANDS). He's seeing an internationally recognized pediatric neurologist at Cincinnati Children's hospital who should be able to determine if he has PANDAS or TS or something else, so I am not switching anything until that appointment in June. I'm also waiting for the website to be updated enough so that I can have my pediatrician review the information and maybe get " on board " with Dr. G's protocol or give me a referral. I've been following this listserv for a short time. I have a few questions to the veterans: 1. Are any of your children drug free and recovered? How long, on average, do these children have to be on these types of meds? One thing that appeals to me with NID protocol is finally getting my son off supplements in future. 2. I'm really curious about the probiotics. I really like them but it looks like they are not recommended on Dr. G's protocol. I think my son has greatly benefited from these. He has used a super-potent probiotic (VSL#3). Since using this probiotic, he has no GI issues now, absorbs foods and is growing better (after years of failure to thrive), no yeast, lower bacteria, lower oxalates and great health. This winter, he was my only child (of 4) who did not get the rotavirus--and my other children had 3 rounds of vomiting!! He shares a bedroom with them and even a water bottle with one of them and still did not get these viruses. It was amazing. He almost never gets sick. Therefore, I'm really hesistant to ever stop using a probiotic. 3. Do any of you know when the site will be fully updated? It has a lot of broken links and old forms. Another thing that I noticed is that the advisory board list was removed. This is debatable in the field, but our PANDAS didn't rear its ugly head until a year of ThreeLac use (strep strain). For whatever its worth.... BTW...what is Dr. G's spin on PANDAS? Guest guest Posted March 30, 2011 Report Share Posted March 30, 2011 The only Strep species I've heard associated with PANDAS is Streptococcus pyogenes (also called group A strep). This is the agent of strep throat, and is the trigger for rheumatic fever, a heart problem that may also include, Sydenham's chorea (which has a movement disorder similar to PANDAS); and post-streptococcal glomerulonephritis (kidney disease). A very specific antibody to this specific type of streptococcus seems to be part of the causation of PANDAS. Other forms of strep (group B--which can cause life-threatening infections in infants), Group G (formerly streptococcus faecalis, now Enterococcus faecalis), Streptococcus pneumoniae (the agent of usual bacterial pneumonia)--are all part of the genus Streptococcus, but are not part of the PANDAS story. Streptococcus faecalis (now called Enterococcus faecalis) is part of ThreeLac; another strep, Streptococcus thermophilus, is in VLS#3. I've never heard that either could be involved with PANDAS. I've also thought that his tic is related to an overactive innate immune system. Since starting the probiotis, my child is never sick! His tics have worsened in response to several innate immune system stimulators--high-dose probiotics (definitely the SCD yogurt and VSL#3 with its 225 billion CFUs per capsule!), Enhansa (concentrated curcumin that purportedly " wakes up the immune system " ) and exposure to viruses. In a recent publication entitled " Immunopathogenic Mechanisms in Tourette Syndrome " , the authors indicate that recent data show that there is a subset of TS patients whose overactive innate/cell-mediated immune system cause tics/OCD. Fortunately, my son is going to see one of the MDs who authored this paper because I am convinced his problem is immunological. Guest guest Posted March 31, 2011 Report Share Posted March 31, 2011 Um ... one thing about your child never being sick:  ... that's not necessarily a good thing... The times that I was " never sick " were the very worst functioning times I ever had. And I was actually sick - I had chronic mycoplasma pneumonia but it was being treated as 'asthma' - but aside from that I never caught any colds or flu going around. I've had PANDAS since I was 9 although some of the years of my teens and early 20s it was very much Tourette's-like w/verbal behaviors and leaping into the air (I found it all pretty hilarious back then) - except I know it's strep - and mycoplasma pneumonia- related and am now 41 in another rather fierce spell minus the fun parts. (I also have 2 kids 'on the spectrum'.) I don't catch any illness around me except strep and mycoplasma. And when I have those I don't really present with the normal symptoms - instead I have increased cognitive problems (very disabling), much worse ocd and anxiety. But I don't appear sick to anyone. I can't take probiotics, as much as I've tried, as much as I believe in them. It worsens my symptoms half as much as being off antibiotics does. Strep is very highest on the list (I mean Group A), but mycoplasma pne. exacerbates symptoms at about 1/2 the severity (bad enough), and I'd say that some strains of probiotics are just as bad as mycoplasma. I took culturelle one time (l.rhamuses) and was chirping and squeaking and leaping and meowing and flickering all over the place - more tics than I had in my peak years. It was nuts. To be honest, I wouldn't mind, if only it also didn't shut down my brain, because otherwise I feel pretty joyful and hyper in that state and I actually like it - if I could otherwise function. It's wierd to have intense fatigue and be hyper thru the roof at the same time - you tell people you're exhausted while you're hopping up and down and they give you this look...  Imagine sitting in a doctors office w/?Tourette's/PANDAS as well as CFS, telling your doctor how tired you are while you burst out laughing at the one of the 20 thoughts that just ran through your brain while you're also talking to him, and you leap up onto the exam table and bounce a moment, giggle again because a silly song is playing too... geez. And at no point did I ever mention my brain racing, the OCD, the 15 different songs playing at one time, the 30 thoughts going on at the same time, because I had no earthly idea that it's not normal because I'd never experienced anything otherwise. When I had my first pregnancy at 30, the immune suppression silenced all that. (It had already been much calmer the few years before - I figured I was just 'growing up'.) I thought something was wrong then, not before. I really had no idea. I'd just thought I had an anxiety problem (duh) and a 'little' ADHD. Other than that, I was just wierd. Now, though, I'm acutely aware of all the differences by what sets it off and what doesn't. I can tell that diet is a big factor too, and I was never worse than when I took supplements and vitamins and ate " healthy " - I worked w/nutritionists and had that perfect healthy diet and was crazier than the nuts I ate and exhausted the whole time. I just wanted to share all that, because sometimes others can't see the internal symptoms at all in order to know that something is setting off our system. I've watched the list for years and years, and PANDAS kids are the most susceptible to the probiotics. I think if you start them early enough and do all the other things to keep the immune system calm, they can be used, but you really have to watch the strains. It also seems to take about 2 weeks to really reach it's peak, and by that time a lot of people have long stopped suspecting the addition of a probiotic. Strep is very highest on the list (I mean Group A), but mycoplasma pne. exacerbates symptoms at about 1/2 the severity (bad enough), and I'd say that some strains of probiotics are just as bad as mycoplasma. I took culturelle one time (l.rhamuses) and was chirping and squeaking and leaping and meowing and flickering all over the place - more tics than I had in my peak years. It was nuts. To be honest, I wouldn't mind, if only it also didn't shut down my brain, because otherwise I feel pretty joyful and hyper in that state and I actually like it - if I could otherwise function. It's wierd to have intense fatigue and be hyper thru the roof at the same time - you tell people you're exhausted while you're hopping up and down and they give you this look... Imagine sitting in a doctors office w/?Tourette's/PANDAS as well as CFS, telling your doctor how tired you are while you burst out laughing at the one of the 20 thoughts that just ran through your brain while you're also talking to him, and you leap up onto the exam table and bounce a moment, giggle again because a silly song is playing too... geez. And at no point did I ever mention my brain racing, the OCD, the 15 different songs playing at one time, the 30 thoughts going on at the same time, because I had no earthly idea that it's not normal because I'd never experienced anything otherwise. I do believe that my son's tic exacerbations in the last 2 years are related to something immunological as you described below. I also think he is sensitive to various " good " bacterial strains or potencies too. I wish we had an MD like Dr. G years ago to evaluate his symptoms from the immunological perspective. Previously, he was always sick and now he's reversed--probably from 2 years of probiotics and dietary changes. Have you tried the L. acidophilus or Bifidium strains of probiotics? I am nervous about totally stopping probiotics because my son had clinically significant GI issues (malabsorption, failure to thrive, inflammation) that have been totally reversed in the past 2 years. With the exception of the tic/OCD, he's indistinct from his peers without any of the autism-like symptoms. I live on antbiotics and struggle with strep, lyme, babesia, bartonella and who knows what else. I have the CFS, fybromyalgia (completely controlled with antibiotics) and arthritis (also controlled) symptoms. They anxiety and mood swings are also controlled with augmentin. However, I do not have ADD or hyperactive behaviors, although do remember having these at different times in my life. I am wondering if the hyperactivity/ADD is related to diet or yeast as I have been a low carb dieter for years. The only time that I actually beat my strep was after being in the hospital with sepsis a year and half ago. In the hospital I was given IV vanco, rocephin and doxy. My strawberry tongue, green tongue coating, tonsilitis and PANDAS symtoms were all resolved for about 3 weeks then they came back. I wish that drs were open to treating our bacterial infections. I have been on oral antibiotics for over 3 years and they do not eliminate these infections. I think our kids need IV vancomycin. Guest guest Posted April 2, 2011 Report Share Posted April 2, 2011 I don't really want to urge anyone to stop probiotics - I do worry about posting my problem with them because I don't want to discourage people from using them. My issue could be specific to me (well, me and one of my kids). I could be more sensitive to them because of more exposure pre-. I just want people to know about the possibility of OCD/anxiety and show why Dr G says to choose kyodophilus (what Dr G recommends) or other one to three strains of bacteria. I really think there are very few kids who can't tolerate them, and sometimes, you might be better off having the symptoms for a while than doing without probiotics. And I will actually try them yet again. I'm tolerating iron a little better now than I ever did, so maybe I'll tolerate kyodophilus better when I'm willing to risk trying it again. I'll know in 2-3 days, and I'll test it 3 or 4 times to rule out my placebo effect. So please take my warnings about probiotics with a grain of salt and don't abandon them on my experience - just be aware with PANDAS.  My experience has been, unfortunately, that PANDAS isn't going to typically just 'go away', even with good treatment, but it will get significantly better with the right antibiotics and antifungals ( & maybe antivirals are addressing it over the long term or thru helping the natural killer cells). It's going to fluctuate, and you can't always find the trigger or what made it better, because sometimes it's just part of the cycle - so I don't abandon anything without testing it over and over again. (Except culturelle - some things are so dramatic they burn into your brain lol - but I was even stupid enough to try that one a second time - just to be sure.) With probiotics, even if they do make someone flicker a little more, it may not always be worth removing - benefits outweighing the negatives. Thanks, ________________________________ From: hildy gogal <hildygogal@...> Sent: Fri, April 1, 2011 12:18:07 PM Subject: Re: Re: new member--tics and probiotics and practitioners  Esther,  What brand of probiotic do you give your son? I too am nervous to stop probiotics with my daughter.  Thanks, Hildy From: ebrooksasplund <esther.asplund@...> Subject: Re: new member--tics and probiotics and practitioners Date: Friday, April 1, 2011, 10:38 AM  Dear , Wow--thanks for sharing your experience. We've also used Klaire's Thera-biotic, which is a multispectrum probiotic, but much less potent than VSL#3. He really didn't have tic problems with he was using Thera-biotic because it is a lower dose (~20CFUs) and has less S. thermophilus. Then we jumped to VSL#3 with its 225 billion CFUs---nearly a 10-fold increase in potency. However, the Therabiotic wasn't enough to control the yeast and clostridia....so I need something in between (maybe). I have read a good deal about PANDAS and PITANDS and TS--a lot of common overlaps. Researchers are now seeing that some Tourette's cases are immunologically triggered. You said you were using antivirals and antibiotics for control. If you are PANDAS, did you ever consider seeking someone to administer IVIG or plasmapheresis? Just wondering. However, I know this is still controversial. Guest guest Posted April 3, 2011 Report Share Posted April 3, 2011 Hi Esther, No I've never looked into IVIG for PANDAS/PITANDS. I think about plasmaresis sometimes, just because I'd love to get this aso enzyme out, and I used to be able to estimate my aso level with surprising accuracy by my symptoms, but plasmaresis is really just a temporary fix, although I think maybe I'd try it if it were offered to me - I don't know. I'm not really going to seek it out though.   With IVIG, there are too many risks involved using a blood product like that - hypothesized right now, but a few small studies have suggested that the hypothesis may be real. I can't quote them, I saw them a few years ago, and I've heard that some immunologists (couldn't say who it was) had started seeing some of the things they had initially been concerned about starting to happen. I can't give you details or say who, because I don't remember - I only remember the discussion. They're related to risks from blood transfusions, and the possibility of auto-immune responses years after the infusions. My doomsayer ocd says if anyone will have the negative response it'll be me. I'm just not hearing enough about long term health improvements with it - I hear plenty of people using it, and while it helps a portion of the patients to some degree, it doesn't seem to be the long-term fix we need, certainly not in comparison to the costs, the side effects, and the risks. I would consider IMGG though. It seems like Dr G doesn't use that much anymore and I'm curious as to why. I'm also interested in immunovir, although I can't afford it right now so that's a moot point. And I don't know if my doctor would be willing to use it if I could. I have low NKs, which I think are a big factor in this illness, at least for me. I had hoped the antivirals would have more impact on that - they did with both of my kids, but so far not me if I'm remembering right. My biggest improvement so far has come from taking amantadine, not part of the protocol, but having a big impact on my cognitive function independent of my ASO levels - the first thing that's done that. But I don't know yet what the long term effect or benefit will be, if it'll wear off or lose it's efficacy or what. I have headaches as a side effect (and I suspect more all over sense of body pain), but I'd rather my brain hurt than not function.   And then there's the ever-necessary anxiety meds. Every time I try to stop those, the whole world comes apart, my brain along with it, but I don't notice right away. I'm rambling... ________________________________ From: ebrooksasplund <esther.asplund@...> Sent: Sat, April 2, 2011 8:21:42 PM Subject: Re: new member--tics and probiotics and practitioners  Thanks again .
