I sent Becky a copy but if you would like to e-mail me personnallly, I can send you a copy of the ones I have found as attachments. Otherwise go to the internet and google in Pain Diary. Let me know If I can help. Doctors seem to take a patient more seriously when they have taken time write down their pain levels and what interventions work and don't. Let me know.
