hi there guys. i am just about to send in all my paperwork and i want to prepare

mmyself (as much as possible) for what kind of initial costs ill have when i go

to take cormac to dr g. i have anthem blue cross ppo thru my employer. fedex

express. its a self funded plan if that helps any...thanks for any input you may
