Pain patients need to keep a pain diary and functional assessment also. There are articles that have been saved on this site that have copies of the pain dairies and functional assessment. This way the doctor can see what works for the pain, how long the pain is covered for with what medications. My doctor requires I keep one and it has helped in my SSI disability application and for other verifications. The National Foundation for the Treatment of Pain has an intractable pain questionnaire that is excellent, the American Pain Foundation has all these templates and follows legislature laws and bills that pertain to pain. It is a good habit to document your pain in a journal or templates provided on the internet. All you have to do is type in pain dairy. Dr. Forest Tenent wrote, The intractactable Pain Patients Guide to Survival, and can be downloaded at Pain Topics.org This is one of the best resources and this site is also I have ever encountered. I encourage everyone to download it and read it. This site also has resources specific to pain patients which is from all organizations. It includes diaries,flow charts, information, articles, clinical trials etc. It has everything in one place. I have used it often and was reminded of it when you guys mentioned it. Hope it helps.
