Bren - Fibro talk, Partners Against Pain, CVS Healthsource Health Mark, American Pain Foundation (Pain Notebook) all have great pain diarys to use. I e-mailed your private e-mail with attachments to see what they look like. You can type in pain diary in Google and you will see many to choose from. Hope that helps all. I use different ones at different times. Let me know. I also have to do a functional assessment which lets my doctor know what physical activities I can do or cannot do.
