Researchers at Harvard Medical School and s Hopkins School of Public

Health are conducting a study to understand patient communication with

physicians by e-mail. If you agree to participate, you will be asked to

complete a short online survey. The survey will ask you about your interest

in communicating with your physician by e-mail and whether you have used

e-mail to contact your physician.

We will not link information from this survey directly to your name, e-mail

or other unique personal information. Confidentiality of your answers will

be guarded as much as possible. We will not discuss your information with

others including your physician.

If you have communicated with your physician through e-mail, we would also

like you to participate in a follow-up interview. You will be asked to enter

your e-mail address if you agree to participate in the second stage of this

study.

Risks/Discomforts

Risks to you are minimal. You may find the survey to be inconvenient.

Confidentiality will be carefully guarded.

Benefits

There will be no direct benefits to you for participating. We will provide a

summary of the results for individuals who participate. This research may

lead to improved e-mail communication in the future. If you have any
