For my state, you request a waiver/exemption form from the state department of health (can do this online) and then it gets mailed to you. I fill it out, get it notarized and give it to the school. I can check off that I am declining all the vaccines. Each state has their own rules, so go to your state department of health and ask for a waiver/exemption and then you will see what your state's rules are. You don't need a waiver/exemption for your doc, only your school. Some docs are ok with not vaccinating, but sometimes people get a new doc when they aren't. I have seen posts on this Internet where parents claimed that nurses gave vaccines even after the parents said they didn't want them; it's so routine that the nurses forget. I have even heard of people yanking their kids away from a nurse who was demanding to vaccinate. So, keep your child in your sight when you go to a healthcare facility. From there you can learn your current laws/requirements & get forms & other exemption paper work/requirements. Guest guest Posted October 27, 2011 Report Share Posted October 27, 2011 Like one of the other ladies posted, you don't need any forms for the doctor. Just tell him no. He most likely will not like that, so you might have to find a new doc. Or be prepared to fight every time you go to see him/her. I finally found a great new doc after I got tired of the guilt trips every time I took my son to the doctor. I have way to many pans in the oven to have to deal with that stress. I just moved onon. Great advice from the ladies about never letting your chchild out of your sight when in a medical facility. I have heard too many stories of babies being vaccinated even after the nurses were told that the parents didn't want them vaccinated.
