RTA phone number - is 1300 366311 The Landlords Intention to leave form is 12—there is a section- 5- with or without grounds-- IF there is reason – (grounds) you can still dispute) however best to do that before if you can as with mediation/ in dispute resolution. The tenants notice to leave form is what you put in if you are not happy with out come after dispute resolution – this is form 13 Before you are forced to or move to quite- these are the things to do – in order Remedy to breach is form 11—is what either party send 1st- to get something fixed- etc A dispute resolution form- ( not mediation but is same as) form 16-- IF all fails (QCAT) Q’land Civil Administration Tribunal—Has a QCAT form 2 AND if you need or want to appeal – or get action before or after the tribunal is a QCAT form 8. These tribunals come to town monthly but also can be done by telelink or video conference also so very helpful. QCAT phone number to ask questions is 1300 753228. If you do a Dispute Resolution or QCAT form- also write a cover letter introducing the issue—and what you have done to date etc. Joy I worked in Q’land housing- and with RTA- also was a mediator for a time way back in the early 90’s. In my role as landlord to our residents and when we apply for housing and rentals for them later after their stay with us in supported accommodation- we use all these forms, these services- on behalf of our clients- Thus we at our organisation-GEMS & I am pretty familiar with them all-- When taking on a rental you receive a green booklet- A4 size Renting a house, unit etc in Qland—it is available online at RTA site- item /form 17—this book covers everything—everything you should ever need-- Hope this is of use to you Joy good luck and lets know how you go—IF it was me in your place I would also attach to all you do—a copy of a letter from your GP or your lung specialists too—a TDR—or similar—I have the forms- just cannot remember the actual name- TDR is for centrelink- there is an equivalent in housing Medical Information Record—I can send a few copies to you- or you can get from Dept of Housing and Homeless site- 3 pages if I remember- must be signed by your GP—being you have bronchiectasis and others conditions- and your health is being extremely affected I would be sure to add this letter or form.
